IF MORE THAN ONE CHILD IS COMING FROM THE SAME FAMILY PLEASE COMPLETE AN INDIVIDUAL FORM FOR EACH CHILD. !NOT TOGETHER! 

Please ensure that your child is aware of their exact order on the evening.

There will be no extra orders allowed during the night.

The Child will only need to bring money for Juliano’s, which they will pay themselves.

(Coaches/Volunteers will be on hand to help the younger juniors with their orders)

Order and Payment - The Order and Payment Needs to be made by Friday 13th December. 

Payment is to be made by BANK TRANSFER ONLY.

This should be paid to Barry’s Account NOT the Ballymena LTC one.
